Gainesville Neurology would like to welcome you to our practice. Patients can download the forms on this page we need before you are seen in our office. Completing the forms prior to your arrival for your appointment will help us treat you in a timely manner. If you are a new patient you should bring any labs, x-rays, images or test that you may have had in the past to your appointment. Also you should complete the Patient Information form, the Past Medical History form, and the Medication Policy form prior to arriving at our office for your first appointment. If you are an established patient and have not been in for a visit in a year you should complete the Patient information form or if there have been any changes in your address, insurance since you were last seen. In addition you should complete the Past Medical History form if there have been any changes in your medical history, medications, allergies, etc. since your last visit. When you have completed the necessary forms, please fax them to us at 770.503.7285. If you do not have access to a fax machine, please bring the forms with you when you come in for your appointment along with a picture ID.
