We are always happy to see your patients. Please call our appointment line at 770-534-7885 to schedule an appointment. We will make every effort to see your patient in a timely manner. If the next appointment is not soon enough, feel free to contact one of our physicians to have your patient seen sooner.
You can also download an EEG / EMG Requisition form below that allows you to specify the indication for EEG / EMG referrals. In addition, the form contains information that we need to set up the appointment and do the pre certification if needed by the patient's insurance.
